1) The initial term of this contract shall be
from _______________ through ________________, with ______ visits a day. In
the event of early return home, Client must notify Pet Sitter promptly to avoid
being charged for unnecessary visit(s).
2) The fee per Visit $_______X________(#
of visits), plus any assessed fees $______= TOTAL FEE of $__________.
Any additional visits made or services performed shall be paid for at the agreed
contract rate.
3)
Pet Sitter is authorized to perform care and services as outlined on this
contract. Pet Sitter is also authorized by signature below to seek emergency
veterinary care with release from all liabilities related to transportation,
treatment, and expense, Should specified veterinarian be unavailable, Pet Sitter
is authorized to approve medical and/or emergency treatment (excluding
euthanasia) as recommended by a veterinarian. Client agrees to reimburse Pet
Sitter/Company for expense incurred, plus any additional fees for attending to
this need or any expenses incurred for any other home/food/supplies needed.
4) In the event
of inclement weather or natural disaster, Pet Sitter is entrusted to use best
judgment in caring for pet(s) and home, Pet Sitter/Company will be held harmless
for consequences related to such decisions.

Pet Sitter agrees to provide the services stated in this contract in a reliable,
caring and trustworthy manner. In consideration of these services and as an
express condition thereof, the Client expressly waives and relinquishes any and
all claims against said Pet Sitter/Company except those arising from negligence
or willful misconduct on the part of the Pet Sitter/Company.
6) Client understands
and takes full responsibility for PROMPT payment of fees upon completion of
services contracted. A finance charge of 10% per month will be added to
unpaid balances after thirty (30) days, A handling fee ($25) will be charged on
all returned checks. One-half deposit is required on lengthy assignments and
first time clients or clients with a history of late payment will be required to
pay in advance before services are rendered. In the event it is necessary to
initiate collection proceedings on the account, Client will be responsible for
all attorney's fees and costs of collection.
7)
In the event of personal emergency or illness of Pet Sitter, Client authorizes
Pet Sitter to arrange for another qualified person to fulfill responsibilities
as set forth on this contract. Client will be notified in such a case, if at all
possible. Client also understands that someone other than Pet Sitter may
occasionally accompany Pet Sitter for training purposes, emergency backup and
assistance. This person is carefully screened and is covered by Pet
Sitter's insurance and bonding.
8) All pets are to be currently vaccinated. Should Pet Sitter be
bitten or otherwise exposed to any disease or ailment received from Client's
animal which has not been properly and currently vaccinated, it will be the
Client's responsibility to pay all costs and damages incurred by the victim.
9)
Pet Sitter/Company reserves the right to terminate this contract at any time
before or during its term if Pet Sitter/Company, in its sole discretion,
determines that a danger exists to the health or safety of Pet Sitter. If
concerns prohibit Pet Sitter from caring for pet, Client authorizes pet to be
placed in a kennel, with all charges wherefrom to be charged to client.
10) Client
authorizes this signed contract to be valid approval for future services of any
purpose provided by this contract permitting Pet Sitter/Company to accept
telephone reservations for service and enter premises without additional signed
contracts or written authorization.
11) Pet Sitter/Company is covered by a dishonesty bond and commercial
liability coverage. With reciprocal consideration, Client agrees to be
responsible for any injuries incurred by Pet Sitter due to Client’s pet(s) or
condition of Client’s premises.
12) SECURITY:
Your house must be securely locked at all times during your absence. We will
not accept jobs where doors are left physically open or unlocked. This is for
the protection and safety of your home, your pets, and the pet sitter.
13) NOTIFICATIONS: We request that you give us a call and leave a voice
message to confirm service 24 to 48 hours prior to your departure. When
you return home, we ask that you notify us of your safe arrival ASAP. Our
business phone is available 24/7. If you arrive home, but do not let us
know, you will be charged for the next scheduled visit.
ADDITIONAL
FEES: An additional fee of $5.00 will be applied to the following
Major Holidays (i.e. New Year's Eve., New Year's Day, Easter, Memorial Day, 4th
of July, Labor Day, Thanksgiving Day, Christmas Eve., and Christmas Day).
If
Client schedules for service on any of the afore mentioned holidays, or during Spring Break, and cancels less than one week prior to the dates for service, a
$10.00 fee will be incurred for each cancelled visit.
A flat rate of
$20.00 per hour will be charged for services such as emergency and non-emergency
veterinarian trips, shopping, and pet taxi services.
Additional fees will be added for additional pets, medicines, plants, excessive
cleaning, excessive mileage, etc. A $15.00 new client set up fee will be
charged, but will be waved if client uses our services.
All
fees are subject to change, and will be posted on the website.
